Add to Cart and Conversion Rate is populated more than 100% in the insight report/product grid.
Debugging Steps
To investigate the issue and the reason, follow the steps below:

Pixel Integration
Check if the pixel is proper on the site.
Check in the pixel monitor that there are no pixel alerts. This issue occurs due to the ATC pixel not being fired correctly.

If pixels are fine, raise a ticket on the Bloomreach Support Portal.
